While appointments vary between individuals, functional care coordination typically begins with a relaxed conversation about your story and your pain. Once we understand what truly matters to you, and what support will best fit your lifestyle, you'll be guided to identify some practical and impactful goals for your care. Then we begin "care planning". Informed by your goals, your practitioner with guide you through the most targeted treatment options and lifestyle interventions available for your condition. You'll create a formalised plan — which will then be provided to your doctor or specialist — and finish with a clear pathway forward — no exceptions.
Functional medicine has two defining points of difference from conventional or "general" medicine. The first, functional medicine looks at the "whole person" — and the interconnections between every biological, psychological and social factor — to understand the complete picture an individual's illness (in this case, pain). We typically recommend a 3-5 day gap between your initial care coordination session, and your medical appointment. This provides your care coordinator and medical provider the time required to collaborate, thoroughly review your case notes together and ensure seamless continuity of care.
