How we help

Our whole-person care pathway is not merely a series of appointments. It's an experiential turning point.

HOW WE HELP

Partner with us to co-create a future that moves beyond pain, limits and loss — to experiences that are more full, more enriching, and more meaningful.

Together, we'll create your whole-person care plan designed to close the gap between the life you imagine for yourself, and the life you're living now.

Then you'll progress at your own pace, with expert guidance from our team, to reach your goals through your personalised regime of therapies, techniques and treatments.

STEP ONE

Begin your journey with functional care coordination.

Understanding what matters to you is essential, so this is where we begin.

Diving into your story and learning about your life, we'll take the time to understand your unique needs, preferences and challenges with pain. This lays our foundation — enabling us to create highly personalised goals for your care and a comprehensive plan to achieve them.

STEP FOUR

Achieve progress, restore resilience and reclaim the life you want to live.

Monitor your progress, make adjustments and achieve results.

As you begin returning to the activities that bring purpose and meaning to your life — and supporting your overall wellness — you'll be reunited with the freedom of resilient health and independence.

STEP TWO

Apply functional pain medicine to treat the source of your pain.

Experience a more personal, thorough and progressive approach to pain medicine.

Prepared through collaboration with your functional care coordinator — and focused entirely on your unique pain goals — your doctor will further build upon the care plan. Diving into diagnostics and learning about your treatment options, you'll receive the expert support you need to make confident and informed medical decisions.

STEP THREE

Begin your whole-person care plan.

Now you have a clear path forward, it's time for action.

Turning plans to into action, you'll integrate targeted therapies, medical support, lifestyle changes and self-care strategies — all curated specifically to achieve the goals that matter most to you.

WHAT SHOULD i EXPECT FROM FUNCTIONAL CARE COORDINATION?

While appointments vary between individuals, functional care coordination typically begins with a relaxed conversation about your story and your pain. Once we understand what truly matters to you, and what support will best fit your lifestyle, you'll be guided to identify some practical and impactful goals for your care. Then we begin "care planning". Informed by your goals, your practitioner with guide you through the most targeted treatment options and lifestyle interventions available for your condition. You'll create a formalised plan — which will then be provided to your doctor or specialist — and finish with a clear pathway forward — no exceptions.

WHAT'S THE DIFFERENCE BETWEEN "FUNCTIONAL" AND "GENERAL" MEDICINE

Functional medicine has two defining points of difference from conventional or "general" medicine. The first, functional medicine looks at the "whole person" — and the interconnections between every biological, psychological and social factor — to understand the complete picture an individual's illness (in this case, pain). Second, functional medicine aims to identify and address the root causes, or underlying "drivers", of disease — thereby treating the condition itself, not merely masking symptoms.

WHY SEE BOTH A CARE COORDINATOR AND A MEDICAL PROVIDER?

A major shortfall in conventional healthcare is the lack of alignment between the treatments offered by practitioners and the patient's personal goals. Functional care coordination aims to solve this problem. By first taking the time to understand you and your life, our care coordinators collaborate with you to construct a care plan designed specifically to help you live the life you want to live. While medical care is often essential to treat the pain itself, it's care coordination that sets you up with a clear plan — integrating specific allied therapies, targeted lifestyle changes and self-care strategies — to begin rebuilding your life.

CAN I SEE A CARE COORDINATOR AND MEDICAL PROVIDER ON THE SAME DAY?

We typically recommend a 3-5 day gap between your initial care coordination session, and your medical appointment. This provides your care coordinator and medical provider the time required to collaborate, thoroughly review your case notes together and ensure seamless continuity of care.
